Thank you for your interest in Physicians Choice Advantage Rx. This plan is designed for people who are entitled to Medicare Part A and enrolled in Medicare Part B who live in Marion or Polk County in Oregon.
We are pleased that you have considered the opportunity for us to provide your Medicare benefits. We know you appreciate the advantage of having a choice.
Physicians Choice Advantage plans are HMO plans, which means you must receive all routine care from plan providers. You may enroll in one of our plans only during certain times of the year or under certain circumstances. For further information about enrollment periods, please call our customer service department at 1-888-462-2708 from 8:00 a.m. to 8 p.m. seven days as week. TTY users should call 1-800-471-2900.

Our Contract with CMS
This contract renews each year. At the end of each year, the contract is reviewed, and either Marion Polk Community Health Plan Advantage, Inc. or CMS can decide to end it. You will get 90 days advance notice in this situation. It is also possible for our contract to end at some other time during the year, too. In these situations we will try to tell you 90 days in advance, but your advance notice may be as little as 30 or fewer days if CMS must end our contract in the middle of the year.
Whenever a Medicare health plan leaves the Medicare program or stops serving your area, you will be provided a special enrollment period to make choices about how you get Medicare, including choosing a Medicare Prescription Drug coverage.
