Plan Benefits
With a low monthly premium of $67.00, this plan includes Medicare Parts A, B, and D coverage as well as supplemental dental, vision and transportation benefits. Based on your medical needs, you can even receive home visits from one of our plan nurse practitioners. This plan does include Part D prescription Drug coverage.
