Because everyone's needs are different, we offer five plans for you to choose from. Our plans are designed to help you lower your costs, or get the prescription drugs you need or give you access to extra benefits to make your life easier.

To enroll in our plans, you must be enrolled in Medicare Part A & B, reside in our service area which is Marion or Polk Counties in Oregon, and cannot be in End Stage Renal Failure. You must continue to pay your Medicare Part B premium.

NEW for 2010 – The Freedom of Choice

Physicians Choice Preferred (PPO)
Physicians Choice Preferred + Rx (PPO)

These plans are designed for those who want the freedom to go to the physician of their choice. We provide reimbursement for all covered benefits regardless of whether they are received in-network, as long as they are medically necessary.

You can select the low cost option of Physicians Choice Preferred to get your Medicare Parts A and B services along with additional benefits, or if you would like to add prescription drug coverage you can enroll in Physicians Choice Preferred + Rx (PPO).

With the exception of emergency or urgent care, it will cost more to get care from out-of-network providers and/or that accessing services from in-network providers can cost less than using services of out-of-network providers. You can get your care from an out-of-network provider, however, that provider must participate in Medicare.

Physicians Choice Advantage (HMO)
Physicians Choice Advantage + Rx (HMO)

Our Physicians Choice Advantage HMO plans have some of the lowest premiums and co-pays in the area. These plans offer comprehensive medical health benefits that cover all services that Medicare covers as well as extra benefits.

Under an HMO plan, you are required to have a Primary Care Provider selected, have referrals from your PCP to see a specialist. You must receive all routine care from in-network plan providers.

For those eligible for Medicaid:

Marion Polk Community Health Plan Advantage

If you qualify for Medicare and Medicaid services in Oregon's Marion and Polk counties, you are eligible for our MPCHP Advantage health plan, a comprehensive medical health plan that includes extra benefits like a home medical alert response system and certain health and wellness classes. This plan includes a drug benefit to give you access to prescriptions for little or no co-pay, depending on the level of extra help you receive.

Under an HMO plan, you are required to have a Primary Care Provider selected, have referrals from your PCP to see a specialist. You must receive all routine care from in-network plan providers.
